Symptom Self-Assessment

This questionnaire covers common motor, non-motor, and daily living symptoms associated with Parkinson's disease. It is designed to help you organize your observations and prepare for conversations with your healthcare team.

How This Tool Works

  • Each question is weighted based on clinical significance and grouped by symptom category (motor, non-motor, daily living).
  • Your results include a score summary with interpretation, organized by category, that you can share with your neurologist or movement disorder specialist.

Not a Diagnostic Tool

This assessment helps you track and communicate your symptoms. It cannot diagnose Parkinson's disease or any other condition. Only a qualified clinician can make a diagnosis based on a full clinical examination. If you are concerned about new or worsening symptoms, contact your healthcare provider.

Understanding Your Results

After completing the assessment, you will receive a score across each symptom category. These scores reflect the frequency and severity of symptoms you reported, not a clinical measurement. Higher scores in a given category suggest that those symptoms are having a greater impact on your daily life and may warrant discussion with your doctor.

No single score is “good” or “bad” in isolation. What matters most is the pattern — which symptoms are most bothersome to you, how they have changed over time, and what areas your healthcare team should focus on. Consider completing this assessment periodically (for example, before each neurologist visit) to track changes.
